Speech Language Pathologist
Responsibilities:
- Make home visits to clients.
- Evaluates patients referred for home care services to determine appropriateness for speech therapy services.
- Establishes a home care plan including assessment of the appropriateness of the requested services.
- Provides rehabilitative services for speech and language disorders to meet established treatment goals.
- Evaluate patients’ conditions for swallowing or cognitive deficits.
- Provides ongoing evaluation of patient’s status and goals, modifying treatment program for timely progression.
- Provides for patient education to include the development of comprehensive and progressive home programs with instruction to the patient, family and other caregivers.
- Keep accurate records of patient’s progress, goals and attendance.
- Selects and use appropriate diagnostic and therapeutic techniques and materials.
- Documents type of treatment and patient’s response on designated clinical progress notes.
Qualifications:
- Master's degree in Speech Therapy or a foreign degree equivalent as accepted by US Citizenship and Immigration Services or a Bachelor's Degree or foreign degree equivalent plus 2 years of progressive, post-baccalaureate experience.
- Valid New York State license as a Speech Language Pathologist.
- Excellent communication skills and the ability to interact well with diverse individuals.
- Basic computer skills.

- Able to function independently, with minimum supervision
- Current Driver’s License, required insurance and car available for work
