Nurse Practitioner / Endocrinology
We are seeking a dedicated and skilled Nurse Practitioner specializing in Endocrinology, to join our healthcare team based in Sacramento, CA. This role offers the opportunity to work with a service-oriented team that commits to the healthcare and improvement of its patients' lives. The Nurse Practitioner will engage in direct patient care, including the assessment and treatment of patients with endocrine conditions, primarily focusing on diabetes management. Compensation: $192,000.00 - $227,000.00 per year
Essential Duties and Responsibilities:
- Practice under the standard procedure established for the NP role consistent with policy and according to the requirements set forth by the California Board of Registered Nursing.
- Conduct comprehensive, age-appropriate assessments of patients, including history taking and physical examinations in line with departmental protocol.
- Participate actively with physicians and other clinicians in patient care, ensuring effective patient outcomes consistent with quality indicators.
- Collaborate extensively with other health care providers and departments to enhance operational efficiency and effectiveness while ensuring continuity of care.
- Monitor patient progress and outcomes; initiate changes in care plan based on patient and family feedback and clinical evaluation.
- Consult with supervising physician or other clinicians as dictated by patient need and assessment findings.
- Delegate tasks and responsibilities to health care team members appropriately, ensuring optimal team functioning and patient care.
- Participate in emergency situations by executing established protocols effectively and documenting events accurately.
- Engage in continuous professional development and participate in unit and department Performance Improvement activities.
- Stay current with advancements in technology and practice, including the use of computers and telecommunication modalities in patient tracking and documentation.
Minimum Position Qualifications:
- Minimum of 6 months experience as a nurse practitioner.
- One year experience within the past three years as a NP managing endocrine conditions.
- Masters Program graduation in Adult NP, Family Practice NP or Pediatric NP as applicable.
- Possession of a Nurse Practitioner Furnishing Number, Certificate, and a Registered Nurse License in California.
- BLS for Healthcare Providers certified by the American Heart Association.
- National Provider Identifier must be obtained at hire.
Preferred Qualifications:
- Solid experience caring for Type 1 diabetes patients.
- Experience in the application of insulin pumps and CGMS software for diabetes management.
- Certified Diabetes Educator (CDE) credentials are highly preferred.
- Demonstrated ability to meet credentialing requirements such as a National Certification from renowned organizations.
