JOB DESCRIPTION
TITLE: RN – Patient Logistics
REPORTS TO POSITION: Nurse Manager Patient Logistics
DEPARTMENT: Transfer and Dispatch Center
POSITION OVERVIEW: The RN - Patient Logistics is responsible for oversight and coordination of patient acceptance, placement and movement to an appropriate St. Charles Health System hospital in accordance with clinical standards and regulations, ensuring patients meet requirements for acute care. Supports Transfer Center outgoing transfers as needed. Collaborates with physicians and nursing leadership to centrally coordinate bed management and patient flow, facilitating quality patient care. Acts as liaison between community partners, providers, patient care departments, and patient registration. Efficiently manage all hospital transfers in a timely manner, prioritizing based on patient need and resources. Consult with on-call providers regarding patient care issues and escalate as appropriate to leadership team and duty administrator. Supports dispatch services. This position does not directly manage any other caregivers.
ESSENTIAL FUNCTIONS AND DUTIES:
- Oversees dispatch operations and acts as a resource for support and problem solving.
- Responsible for overseeing coordination of direct admissions and hospital transfers.
- Oversees coordination of inter-facility patient transports and collaborates with transfer/dispatch for timely patient transportation.
- Facilitates patient transfer requests and maintains communication with all parties involved.
- Maintains oversight of transfers and prioritize according to acuity and patient need.
- Communicates with providers regarding patient placement issues and assists in problem-solving.
- Facilitates accurate data collection and analysis for process improvement.
- Exhibits professional customer service skills in all communications.
- Establishes awareness of protocols and services at St. Charles Health System facilities.
- Prioritizes decision making for patient flow based on various factors.
- Ensures patient safety and supports accountability for safety behaviors.
- Collaborates with Administrator on Duty and patient care departments regarding patient placement delays.
- Coordinates with environmental services for efficient bed turnaround times.
- Maintains collaborative relationships with managers and clinical leadership.
- Ensures compliance with state, federal, and Joint Commission guidelines.
- Supports the organization’s vision, mission, and values.
- May perform additional duties as required or assigned.
EDUCATION:
Required: As required by licensure.
Preferred: BSN
LICENSURE/CERTIFICATION/REGISTRATION:
Required: Current Oregon RN license. AHA Basic Life Support for Health Care Providers Certification.
EXPERIENCE:
Required: Three years progressive acute care clinical experience. Knowledge of EMTALA regulations.
Preferred: Critical care and ER nursing experience. Bed Tracking Software and EPIC experience.
PERSONAL PROTECTIVE EQUIPMENT:
Must be able to wear appropriate Personal Protective Equipment (PPE) required to perform the job safely.
PHYSICAL REQUIREMENTS:
Continually (75% or more): Standing and walking, keyboard operation, clear speaking voice, and ability to hear normal speech level. Frequently (50%): Sitting, lifting/carrying/pushing or pulling 1-25 pounds. Occasionally (25%): Bending, stooping/kneeling/crouching, reaching overhead, grasping/squeezing. Rarely (10%): Climbing stairs. Never (0%): Climbing ladder/step-stool, lifting/carrying/pushing or pulling 25-50 pounds.
Exposure to Elemental Factors: Rarely (10%): Wet/slippery area, chemical solution. Never (0%): Heat, cold, noise, dust, vibration, uneven surface.
