Compensation: $40.00-$58.67 (DOE). Differentials $5/hour nights & $2/hour weekend.
Sign-on Bonus: $2,000 paid after 90 days of employment. Location: Redmond and Bend positions available.
Schedule(s) Available: Full-Time: Monday-Friday 8:00 am-5:00 pm.
Travel: Local/regional travel by personal vehicle during the working day. Mileage reimbursement is provided.

What You Get To Do:
- Complete an initial comprehensive and accurate assessment of the patient and family to determine health needs.
- Develop and update individualized care plans, establishing goals based on nursing diagnosis and incorporating therapeutic, preventive, and rehabilitative nursing actions.
- Coordinate appropriate preventive and rehabilitative nursing procedures, treatments, medication, equipment, and end-of-life comfort procedures as the physician prescribes.
- Provide health care instructions and education to the patient and/or family as appropriate per the assessment plan and the patients’ caregivers’ ability to learn.
- Identify discharge-planning needs as part of the initial care plan development and implement them before the patient's discharge.
- Develop, monitor, and update the Home Care Aid care plan as required.
- Communicate with all healthcare team members to promote coordinated, efficient care and document coordination of care with all involved with patient care.
- Participate in weeknight and weekend on-call duties as defined by the on-call policy.
Required Skills & Qualifications:
- Valid Oregon RN License.
- Ability to pass a background test, drug screen, and TB test.
- Graduate of an accredited nursing school.
- One to two years of recent medical care experience in an institutional setting is preferred. Ideal candidates will possess 1-2 years of home health or hospice experience.
- Current automobile insurance is required. A valid Oregon driver’s license and reliable transportation.
- Valid CPR Certification.
- Strong communication and interpersonal skills.
- Ability to work independently and collaboratively within an interdisciplinary team.
