About the Role:
We are seeking a compassionate new graduate RN to join our Home Health team. HCR RNs provide high-quality nursing care to patients in the comfort of their own homes. This is a rewarding opportunity to make a positive impact on the lives of individuals in need.
Responsibilities:
- Conduct comprehensive assessments of patients' health status and develop individualized care plans.
- Administer medications, treatments, and therapies as prescribed by physicians.
- Monitor patients' vital signs and symptoms, and report any changes or concerns to the appropriate healthcare professionals.
- Provide education and support to patients and their families regarding their medical conditions, treatment plans, and self-care techniques.
- Collaborate with interdisciplinary healthcare team members to ensure coordinated and effective patient care.
- Document all patient interactions, assessments, interventions, and outcomes accurately and timely on an EMR.
- Adhere to all applicable laws, regulations, and ethical standards in the provision of nursing care.
Qualifications:
- Current, valid RN license issued by NYS Department of Education required.
- Excellent communication and critical thinking skills.
- Fluency and solid comfort level with personal computer operations (email, MS Teams, EMR systems).
- Comfortable with online learning management systems.
- Valid Driver's license and reliable transportation.
- Shift availability: Day Shift (Required).
- Willingness to travel: 100%.

Ability to Relocate: Onondaga County.
Pay range and compensation package:
From $73,000.00 per year.
Mileage: 65.5 cents per mile.
Incentive Plan (Earn extra $$ - unlimited).
