The Registered Nurse promotes and restores patients’ health by completing the nursing process, collaborating with physicians and multidisciplinary team members. Under the direction of the Clinical Manager, acts as a resource for clinical support and management staff. Effectively works with the providers, managers, and staff to promote quality and cost-effective daily operations. Establishes and maintains good interpersonal relationships with patients, families, the public, and co-workers. Assists in procedure set-up, including saline infused sonogram, endosee, IUD insertion, Nexplanon, colposcopy, LEEP, and other gynecological procedures. This position works collaboratively with all other staff to assist the patient and facilitate successful patient interaction with the practice team. This position reflects and carries forward the mission and goals of the practice both internally and throughout the community.
PATIENT CENTERED MEDICAL HOME (PCMH):
Crouse Medical Practice provides services through a team model that supports the patient-centered medical home. Knowledge of the below PCMH elements as they relate to an employee’s specific responsibilities contained in this description is essential.
Engage patients, families, and their caregivers in understanding, establishing, and monitoring patient self-management care plans in a manner that is culturally and linguistically appropriate to the patient, family, and caregiver.
Apply the principles of comprehensive, community-based, patient-centered, developmentally appropriate, and culturally and linguistically sensitive care coordination services.
Coordinate the patient’s care by facilitating patient, family, or caregiver access to medical home providers, employees, and resources as needed by the patient.
Complete health risk assessments as a foundation of developing individualized care plans and outcomes goals for patients, families, and caregivers. Document each patient’s individualized care plan and care coordination in the Practice’s EMR. Conduct and document assessments of patient needs and resources for effective self-care management.
Develop and maintain relationships among patients, families, caregivers, and the patient’s care team that support patients’ access to the medical home.
Act as the primary contact point, advocate, and source of information for patients and the community partners who help treat them.
Research, find, and link patients to resources, services, and support mechanisms for their care plans and self-care needs.
Provide timely communication with patients, make inquiries, execute follow-up actions, and help to integrate information into the care plan.
Assist the care team by helping to measure quality and identify, refine, and implement practice improvements that support the medical home, and assist in performance evaluation and quality improvement.
Seek continuing professional growth through attendance at workshops, professional in-services, and individual research and reading, to include communication skills.
TYPICAL WORKING CONDITIONS & PHYSICAL DEMANDS:
Some exposure to communicable diseases or toxic substances, ionizing radiation, therapeutic preparations, and other conditions common to a health care environment.
Requires full range of motion including handling and lifting, manual and finger dexterity, and eye-hand coordination. Requires standing and walking for extensive periods of time. Responsibilities:
- Understands and implements all nursing procedures, manuals, office policies, protocols, and OSHA Standards.
- Possesses knowledge of medical terminology, office procedures, and insurance requirements.
- Leads the Daily Team Huddle to prevent obstacles and/or challenges from interfering with the patients’ visits.
- Confers with the provider regarding any incomplete patient tests/consults or other incomplete orders prior to the patient visit.
- Responsible for accurately and legibly documenting in the EMR.
- Assists in preparing patients for examination and treatment by interviewing patients to obtain and document health history, medical information, chief complaint, vital signs, and height, weight, and health risk factors. Assesses disease condition. Provides patient care based on practice guidelines, standards of care, and federal/state laws and regulations.
- Demonstrates competency in administering medications and is knowledgeable regarding possible contraindications. Prepares and administers immunizations per provider orders.
- Performs EKGs and other routine diagnostic tests per provider orders.
- Administers dressing changes, applying aseptic techniques, including sterile dressings to ensure infection control per provider orders.
- Assists with specimen collection per provider orders.
- Refills prescriptions per provider orders.
- Participates in population management activities as directed by the practice.
- Performs triage functions.
- Assists with scheduling tests, treatments, and referrals.
- Conducts and documents assessments of patients’ needs and resources for effective self-management.
- Provides safety and health education to individuals and their families per provider orders.
- Maintains a professional and courteous relationship with patients and their families.
- Assists with the preparation of the exam room for patient visits, ensuring that the rooms are stocked, neat, and clean.
- Assists the provider during the examination when needed.
- Keeps the patient informed about their care in a timely manner, as well as, keeps the patient up to date and addresses any concerns.
- Provides timely communication with patients, makes inquiries, executes follow-up actions, and helps to integrate information into care plans.
- In conjunction with the Clinical Manager, anticipates the Team’s daily staffing needs. Assigns additional duties as changes in the schedule warrant.
- Ensures compliance with EMR documentation guidelines and ensures appropriate clinical protocols are followed within the Team.
- Ensures clinical assignments are completed in a timely fashion. Occasionally, will complete special projects.
- Participates in the orientation of new staff. Acts as a resource person to the Team and other Team Leaders.
- Assists with the implementation of practice policies and procedures within the Team. Assists with communicating practice updates and new procedures to members of the Team.
- Provides feedback to the Clinical Manager on Team job performance. Informs Clinical Manager of corrective action needed.
- Maintains professional relationship with fellow employees, physicians, and patients.
- Demonstrates strong work ethic by exhibiting work behaviors consistent or above practice standards for other team members to emulate. Acts as a role model in showing flexibility and demonstrating a willingness to help.
- Ensures the delivery of the Patient Centered Medical Home Standards through a care team approach.
- Patient Care Environment: Demonstrates appropriate knowledge and expectations related to emergency procedures to implement in the event of a medical emergency, violence, fire, disaster, or severe weather.
- Other: This individual is expected to assist in meeting the goals set forth by the Medical Director and Practice Administrator. This will include other duties as assigned by members of the management staff.
MINIMUM REQUIREMENTS:
1- EDUCATION/CERTIFICATION/LICENSURE:
- Required: Current NYS Licensure as a Registered Nurse. Completion of an accredited school of nursing
- Preferred: Bachelor’s Degree of Nursing from an accredited nursing school.
2- EXPERIENCE:
- Required: Minimum of 1 year professional nursing experience
- Preferred: Experience in a physician office
3- SKILLS/PROFICIENCIES:
- Required:
- Knowledge of nursing processes; health care systems, structure, and functions; community resources.
- Knowledge of state law regarding nursing care, nurse practice guidelines, and office policies and procedures.
