Coastal Horizons Center, Inc. seeks a qualified Care Management professional for our TASC (Treatment Accountability for Safer Communities) Justice Services Intervention Program. We have this specific opportunity located in:
Wake County, NC (Garner, NC)
Primary duties include:
- Administering psycho-social screenings/assessments to gather information on client's backgrounds, needs, and challenges.
- Develop individualized service plans in collaboration with clients to address specific goals and objectives.
- Coordinating linkage to behavioral health treatment and medical, educational, and vocational services as needed.
- Advocate for clients' rights and access to services within the justice and behavioral health systems.
- Regularly monitor clients' progress and compliance with recommended treatment services.
- Prepare and submit detailed progress reports to relevant stakeholders, including probation officers.
- Foster a supportive and trusting relationship with clients to encourage their active participation in the care management process.
- Providing ongoing professional care management services for the justice-involved population.
Qualifications:
Education: High school diploma or higher
Certifications: Preference is given to individuals registered with the North Carolina Addiction Specialist Professional Practice Board (NCASPPB) and/or individuals with North Carolina Peer Support Specialist certification and at least two (2) years of experience.
Experience: A minimum of two (2) years of experience working in the justice and/or behavioral health field in addition to the minimum educational requirements. Knowledge of recovery, harm reduction, and wellness-based concepts. May have lived experience with recovery and/or the justice system.
Additional Skills:
- Strong communication and interpersonal skills.
- Ability to work collaboratively with diverse stakeholders, including the Dept. of Adult Correction, behavioral health professionals, and community organizations.
- Familiarity with local community resources and support services.
- Empathy and cultural competence in working with individuals from diverse backgrounds.
- Proficient computer skills, including navigating the internet, word processing and spreadsheet applications, and primary email platforms.
- Bilingual Spanish/English speaking skills are a plus!
Salary: The salary range for this position is $42,000-$45,000 commensurate with experience, medical/dental insurance, life insurance, disability insurance, retirement savings plan/401K, and paid time off programs.
