Sentara Martha Jefferson is hiring a Manager, Inpatient Case Management Registered Nurse in Charlottesville, VA!
The Manager, Inpatient Case Management Registered Nurse is responsible for managing and coordinating the inpatient care transitions program. This ensures the patient's progression through the care continuum in a manner that achieves designed clinical and financial outcomes. Participates in the system development and implementation of integration of care transitions along the continuum of care. Works in close collaboration with the multidisciplinary team, in all settings to include ambulatory, post-acute providers, and health plans. Serves as mentor by role modeling a professional practice consistent with organizational goals, customer service, clinical effectiveness, and standards of care. Responsible for developing and managing the departmental budget, compliance to federal, state, and regulatory requirements.
Requirements:
- 1 year of leadership experience
- 3 years of Case Management Experience- Acute Care.
- Bachelor's degree
- Master's degree preferred.
- Registered Nurse Licensure
- Specialty certification required within one year of eligibility (ACM, CCM, CCCTM, CMAC or CMGT-BC). For other service lines, certification based on specialty area required within one year of eligibility.
- BLS within 90 days of hire.
About the department:
The Integrated Case Management team at Sentara Martha Jefferson has 13 members including MSW and RN Case management staff. This is a collaborative environment with a patient-first mindset. The average cases per day are 15, and a strong sense of urgency is preferable.

Qualifications:
- RN-Bachelor's Level Degree (Required)
- Basic Life Support (BLS) - Certification - American Heart Association (AHA)
- Registered Nurse (RN) - Nursing License - Virginia Department of Health Professionals (VADHP)
