Responsibilities:
- Manage Financial and Clinical Operations of the Case Management team, including RN's and support staff.
- Identify patients of the hospital appropriate for Case Management according to criteria determined by the Case Management Team.
- Monitor and coordinate continuum of care for patients.
- Work collaboratively with the Case Management Team, WRMC staff, physicians, community members, and other service providers to utilize and develop services that most appropriately meet patients' needs, while striving to sustain and/or improve the quality of life.
- Perform utilization review, and participate in medical management program (pathways, protocols, and guidelines) as appropriate.
Qualified candidates will have:
- Bachelor’s Degree, preferred
- Three years of experience in acute care AND supervisory experience
- Licensed in the state of Wisconsin as a Registered Nurse or eligible, required
- Certified in case management
