Summary:
Reporting to Care Dimensions’ Associate Chief Medical Officer (CMO) or the Medical Director, Danvers, the Hospice and Palliative Care Physician (Physician) will be an integral member of the Palliative Care Consultation Team and the Care Dimensions hospice interdisciplinary team (IDT). The Physician has responsibility for the direction of the medical component for assigned hospice and palliative care patients.
How You'll Contribute:
- Participates as a member of the Palliative Care Team in the development and implementation of the plan of care for each patient/family.
- Directs patient medical care and treatment of assigned patients rendered by the Palliative Care Team, and consults and cooperates with the patient’s attending/primary physician.
- Provides pain and symptom management consultation in multiple sites that include patient homes, assisted living facilities, hospitals, nursing homes, clinics and the Care Dimensions’ Hospice Houses, as needed. May assume the role of the patients’ attending/primary physician as appropriate.
- Functions as an active consultant to area providers and facilities, providing clinical expertise regarding diagnosis and appropriate medical management of patient/family needs.
- Educates and serves as a liaison between the Care Dimensions, community physicians and other healthcare providers in order to develop and expand outreach to the professional community by Care Dimensions.
- Makes regular contact with practicing physicians in the Care Dimensions service area to introduce the Hospice and Palliative Care Program, encourage utilization of Care Dimensions services, and answer clinical and other concerns of physicians with respect to Care Dimensions.
- Develops clinical hospice and palliative care standards of practice and protocols to ensure continuity of patient care delivery.
- Prepares such written reports as may be required by state and federal regulations and the needs of the agency.
- Provide physician consultation services for mutually agreed upon long term or home care Care Dimensions team.
- Participate in patient care planning by attending the appropriate IDT meeting and reviewing medical information on Care Dimensions patients.
- Determine patient eligibility for Hospice services in accordance with program admission criteria.
- Certify in conjunction with patients’ attending physicians that patients accepted for admission to Care Dimensions are terminally ill with a prognosis of six months or less.
- Recertify patients’ prognosis and continued eligibility for Hospice services at intervals specified by the Center for Medicare and Medicaid Services (CMS).
- Assume responsibility for the general medical needs of Care Dimensions patients to the extent that these needs are not met by the attending physician and according to patient/family choice.
- Participates in on call rotation and holiday/weekend coverage as necessary to ensure operational needs are met.
- Participate upon request in continuing education programs for Care Dimensions staff, board members, and referral sources.
- Performs other duties assigned as required or requested.
What You'll Bring:
- Possession of a current license to practice medicine in the Commonwealth of Massachusetts.
- Board certified with a minimum of two years experience in Internal Medicine, Family Practice or a related field.
- Privileges or eligible for privileges as an attending physician with one or more Hospice of the North Shore contracted acute care hospitals.
- Successful completion of a palliative care fellowship program strongly preferred.
- Eligibility and/or certification by the American Academy of Hospice and Palliative Medicine (AAHPM) desirable; willingness to obtain certification within two years required.
- Demonstrated competency in symptom control measures and appropriate palliative care for terminally ill patients.
- Demonstrated understanding of and support of an IDT approach to patient care planning and delivery.
- Excellent verbal and written communication skills with individuals and groups.
- Strong interpersonal skills with the ability to function as a member of an interdisciplinary team.
