Position: Family Nurse Practitioner – Health Home Program
Location: Maryland (Forestville, Waldorf, Lanham, Odenton)
Work Schedule: Full-Time (40 hours/week, 8:30 am – 5:00 pm, rotating across multiple locations)
Salary: $52 to $60 per hour (based on experience and licensure)
What You’ll Do:
As a Family Nurse Practitioner in our Health Home Program, you’ll guide somatic care health plans for clients in our PRP and Residential Programs, while collaborating with an interdisciplinary team to ensure holistic care. You will lead the charge in monitoring, assessing, and directing patient care for optimal outcomes, leveraging your expertise in physical health, mental health, substance abuse, and developmental disabilities.
Your role involves rotating between our Maryland locations, where you’ll engage directly with clients, manage health plans, and make a real difference in their lives. You’ll have the autonomy to exercise sound clinical judgment while supporting the goals of Vesta’s integrated care model while obtaining support from our Medical Director.

What We’re Looking For:
We’re searching for an experienced Family Nurse Practitioner who is passionate about integrated, holistic care and has:
- A valid Maryland Nurse Practitioner license with prescriptive authority.
- 5+ years of clinical experience in direct service provision, including mental health and addiction services.
- Expertise in motivational interviewing, cognitive behavioral approaches, trauma-informed care, and evidence-based practices.
- In-depth knowledge of state and federal laws governing behavioral and somatic health care.
- Strong communication skills and the ability to cultivate productive relationships with clients and fellow providers.
- Familiarity with electronic health records (EHR) and data-driven healthcare.
Your Responsibilities:
- Oversee somatic care health plans and collaborate with interdisciplinary teams to promote optimal outcomes.
- Conduct assessments, manage patient care, and implement evidence-based practices across multiple locations.
- Lead care for clients in mental health and residential programs by integrating physical health and behavioral health services.
- Stay current with healthcare regulations and industry standards to ensure compliant and effective care.
- Communicate effectively, both orally and in writing, with clients, families, and other healthcare providers.
