Job Summary: MLee Healthcare is seeking a dedicated Home Health Physical Therapist to join our client's top-tier healthcare team. In this role, you will provide comprehensive physical therapy services to patients in their homes, ensuring the highest level of care and service excellence.
Key Responsibilities:
- Perform patient evaluations to determine physical therapy needs.
- Develop and implement individualized treatment plans.
- Conduct reassessments and adjust treatment plans as necessary.
- Manage OASIS SOCs (Start of Care), ensuring accurate documentation and compliance.
- Provide discharge planning and follow-up care as needed.
- Collaborate with physical therapist assistants to carry out prescribed treatments.
- Maintain clear and detailed patient records.
- Communicate effectively with patients, families, and healthcare team members.
- Educate patients and caregivers on home exercise programs and therapeutic techniques.
Qualifications:
- Licensed Physical Therapist in the state of Ohio.
- Previous experience in home health care preferred.
- Strong understanding of OASIS documentation requirements.
- Excellent communication and interpersonal skills.
- Ability to work independently and manage a flexible schedule.
- Valid driver’s license and reliable transportation.
