NFI North Array of Services, Community Based Services has a full-time Clinical Case Manager: Intensive In Home (ISO) position available in the Concord/Keene/Claremont area with possible travel statewide.
Responsibilities include:
- Partnering with families
- Teaching parenting skills and development
- Case management
- Co-leading family days
- Attending treatment meetings
- Providing transportation
- Writing family assessments, treatment plans, court reports, and monthly reports
- Connecting youth and families to community resources
- Enhancing permanent connections for youth
Clinical Case Managers participate in weekly management and staff meetings, provide on-call support, and work as part of a multi-disciplinary treatment team.
Must be willing to travel. Applicants must have a BA in Social Work, Psychology, or a related field and experience working with youth and families.
