The community you’ll be caring for: AdventHealth East Orlando:
AdventHealth East Orlando, a 265-bed community hospital, has been serving East Orlando residents since 1941 as an innovative local leader that fills a vital need in a fast-growing area.
A recent 200,000 square-foot expansion project upgraded the hospital to 265 beds, with a spacious patient tower and 80 new private rooms designed to enhance the holistic care experience.
Annual number of emergency cases: 89,018.
The role you’ll contribute:
The Behavioral Health Navigator is a Licensed Clinical Social Worker (LCSW) or Licensed Mental Health Counselor (LMHC) who is a key member of the healthcare team working with patients and their families to help manage the difficulties of a hospital stay. The Behavioral Health Navigator utilizes professional social work and therapeutic skills to assess patients for psycho-social and economic barriers that interfere with the attainment of healthcare goals. The Behavioral Health Navigator works closely with the care managers, physicians, nursing staff, and spiritual support for patients and families with counseling services, crisis intervention, patient and parent/spouse coping, bereavement, and substance use issues and continuity of care. Actively participates in outstanding customer service and accepts responsibility in maintaining relationships that are equally respectful to all.
The value you’ll bring to the team:
- Performs behavioral health intakes, psycho-social assessments, care plans, and psychotherapy with patients and families coping with behavioral health disorders.
- Maintains comprehensive community resources and networks to assist patients and families as they transition back to the community.
- Collaborates with the multi-disciplinary team in establishing/documenting psycho-social determinants of health to attain safe discharges and/or remain in the community to avoid emergency hospital admissions.
- Serves as a consultant and educator to the multi-disciplinary team regarding psycho-social components of illness and hospitalization.
- Maintains professional competency by participating in continuing education and appropriate learning experiences, both internally and externally. Attends pertinent conferences and seminars regarding behavioral health.
- Participates in the development of programs to support patients and families dealing with behavioral health disorders.
- Effectively communicates with patients, families, staff, physicians, and community leaders to further development of comprehensive support for the patient.
- Ensures effective communication of pertinent medical information is provided to the patient.
- Attends physician rounds and forums to facilitate communication and behavioral health input to the interdisciplinary team.
- Follows up with patients in the community and/or the post-acute environment to ensure compliance with the hospital discharge plan.
- Initiates Baker Acts when appropriate.
- Performs other duties as assigned.
The expertise and experiences you’ll need to succeed:
- Masters Nursing and 2 years experience.
- Two years' experience in clinical social work.
- Master's Degree in Social Work or Master's Degree in Mental Health Counseling.
- Clinical Social Worker License.
- Licensed Mental Health Counselor.
- Preferred:
- Experience in acute care hospital.
- Experience working with medically complex patients.
- Experience working with substance use and mental health disorders.
- Experience in providing psychotherapy.
- Experience with Baker Acts and Marchman Act.
