GWIN (Go Where Is Needed) RN FLOAT – Position Summary
The GWIN (Go Where Is Needed) Nurse plans, organizes and directs hospice care throughout the following areas of patient care: Inpatient Care Center, On-Call/Afterhours and Admissions after 16:30. Experienced in nursing, with emphasis on community health education/experience. The GWIN Nurse builds from the resources of the community to plan and direct services to meet the needs of the individual and families within their homes and communities.
GWIN (Go Where Is Needed) RN FLOAT – Essential Duties & Responsibilities
Patient Assessment and Care Planning
- Completes initial, comprehensive, and ongoing assessments of patients and family to determine hospice needs. Provides a complete physical assessment and history of current and previous illness(es).
- Uses health assessment data to determine nursing diagnosis.
- Assesses and evaluates patient’s status by writing and initiating a plan of care, regularly re-evaluating patient and family/caregiver needs, and participating in revising the plan of care if necessary.
- Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the physician in the physician’s plan of care.
Clinical Care and Nursing Interventions
- Provides professional nursing care by utilizing all elements of the process.
- Initiates appropriate preventative and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician’s plan of care and/or orders.
- Assists the patient with the activities of daily living and facilitates the patient’s efforts toward self-sufficiency and optimal comfort care as needed.
- Assists the patient and family/caregiver and other team members in providing continuity of care.
- Provides and maintains a safe environment for the patient.
Patient and Family Education and Support
- Counsels the patient and family in meeting nursing and related needs.
- Provides health care instructions to the patient as appropriate per assessment plan.
- Teaches the patient and family/caregiver self-care techniques as appropriate. Provides medication, diet, and other instructions as ordered by the physician and recognizes and utilizes opportunities for health counseling with patients and families/caregivers.
- Works in cooperation with the family/caregiver and hospice Interdisciplinary Group Members to meet the emotional needs of the patient and family/caregiver.
Documentation and Communication
- Completes, maintains, and submits accurate and relevant clinical documentation regarding the patient’s condition and care given. Records pain/symptom management changes/outcomes as appropriate in the EMR at bedside.
- Communicates with the physician and other members of the IDT regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives and documents physicians' orders as required.
- Coordinates arrangements for equipment and other necessary items and services are available.
Interdisciplinary Collaboration and Teamwork
- Must be flexible with daily assignments and able to adjust to last minute needs of the organization.
- Acts as the designated role when assigned by the Vice President of Clinical Operations and assumes responsibility to coordinate patient care for assigned duties.
- Participates in ICC duties as defined by the ICC policy, when assigned.
- Participates in Admission nurse duties as defined by the Admissions Policy, when assigned.
- Participates in On-Call duties as defined by the on-call policy, when assigned.
- Works in concert with the interdisciplinary group.
- Supervises ancillary personnel and delegates responsibilities when required.
- Attends interdisciplinary group meetings.
GWIN (Go Where Is Needed) RN FLOAT – Qualifications/Experience
- Current Registered Nurse (RN) license from the State of Virginia or eligible to practice pending licensure as a Registered Nurse in the Commonwealth of Virginia with either a: Multi-state license, under the Nurse License Compact. BSN preferred.
- Responsible for supervising hospice aides and LPN’s for plan of care for patients.
- Knowledge and understanding of the hospice philosophy and hospice criteria for admission and the admission process.
- Knowledge of private insurance, Medicare as well as State and Federal regulations.
- Ability to document in an EMR system.
- Maintain a current CPR certification.
- Strong Presentation Skills.
- Excellent oral and electronic communication skills.
- Proficient computer skills and electronic medical records required.
- Must be able to travel throughout the territory as needed.
