Job Summary:
Incumbent is responsible to function as a member of the Physical Therapy (PT) department in the rehabilitation and extended care (REC) integrated care community (ICC), providing in-home therapy services to veterans enrolled in the Black Hills VA Health Care System (BHHCS). The primary purpose of the work is direct veteran care with the goal of maximizing the veteran's functional ability. The duties and responsibilities of this position are carried out in the home for the HH population.
BASIC REQUIREMENTS:
- Citizenship: Citizen of the United States.
- Education and Experience: Must meet at least one of the following requirements:
- Bachelor's degree in Physical Therapy AND five (5) years of progressively independent experience as a physical therapist.
- Master's degree in Physical Therapy AND two (2) years of progressively independent experience as a physical therapist.
- Doctorate degree in physical therapy.
- Licensure: Must hold a full, current, and unrestricted license to practice physical therapy in a State, Territory, or Commonwealth of the United States, or in the District of Columbia.
GS-11 (Entry Level) Grade Determinations:
None beyond the basic requirements.
GS-12 (Full Performance Level) Grade Determinations:
In addition to the basic requirements, candidates must possess one year of experience equivalent to the GS-11 grade level, hold a valid unrestricted state license and demonstrate the following:
- Ability to make autonomous clinical decisions in a Direct Access environment.
- Skill in performing examinations and evaluations of individuals with impairments.
- Ability to interpret findings and establish a diagnosis within the scope of physical therapy.
- Ability to provide clinical oversight of Physical Therapy Assistants.
Major Duties:
- Works effectively with treatment team members, veterans, and their families in coordinating care.
- Participates in interdisciplinary team meetings.
- Documents evaluation results and treatment plans in accordance with facility policy.
- Conducts home visits to assess veteran's need for adaptive equipment and modifications.
- Prepares and implements discharge plans that address follow-up resources.
Work Schedule: Monday-Friday, 7:30am-4:00pm
