As an Aging Care Manager 2, you will be working with older adults, their families, and professionals through ongoing care management responsibilities. This role includes specialized duties, to include but not limited to protective services, care transitions, and person-centered counseling. Maintaining a caseload of consumers and involvement in necessary areas of assistance will be essential. You may have the opportunity to regularly coordinate services with physicians, community agencies, and financial institutions. Work Schedule and Additional Information:
- Full-time employment.
- Work hours are 8:00 am to 4:30 pm, Monday - Friday, with 60-minute lunch.
- On-call hours when required.
- Salary: In some cases, the starting salary may be non-negotiable.

QUALIFICATIONS
Minimum Experience and Training Requirements:
- Six months as an Aging Care Manager 1 or a County Caseworker 1;
- Successful completion of the County Social Casework Intern program;
- Six months of professional human services or nursing experience; and a bachelor’s degree in social sciences, behavioral sciences, human services, or a closely related field;
- An equivalent combination of experience and training.

- Hiring preference for this vacancy may be given to candidates who live within Lackawanna, Monroe, Pike, Susquehanna, or Wayne counties. If no eligible candidates who live within these counties apply for this position, candidates who reside in other counties may be considered.

- This position falls under the provisions of the Child Protective Services Law.
