DUTIES AND RESPONSIBILITIES FOR HOME HEALTH SUPERVISOR:
- Receive case referrals. Review available patient information related to case, including home visits, to determine hospice/home health needs. Assign appropriate hospice/home health personnel to case as needed. Confer with the Director regarding any questions about an individual’s eligibility for services.
- Review and evaluate each case through a variety of means such as home visits, conferences, record review, and the services provided by clinicians; discuss and verify impressions, instruct and guide clinicians to promote more effective performance and delivery of quality hospice/home health services; and be available at all times during operative hours to assist clinicians as appropriate.
- Assist clinicians in establishing immediate and long-term therapeutic goals, in setting priorities, and in developing plan of care.
- Attend case conference meetings with hospice/home health personnel to facilitate coordination of care and discussion of interdisciplinary group involvement.
- Assist in the screening and interviewing process of new employees and make recommendations for employment of individuals. Assist in the orientation of new employees.
- Assist Patient Care Coordinator/Clinical Director and other supervisory hospice/home health personnel in the planning, implementation, and evaluation of in-service and continuing education programs. Assist in the formulating, revising, implementing, and evaluating organization policies, procedures, goals, and objectives, both short and long range.
- Assess health needs of the community and assist in the development of plan(s)/program(s) to meet needs. Participate in public relations and community activities that promote the organization’s role as an effective member of the hospice/home health delivery system. Promote customer service orientation to all hospice/home health personnel.
- Participate actively in quality management program.
- Perform other duties and activities as delegated by the Patient Care Coordinator/Clinical Director.
EXPERIENCE AND REQUIREMENTS OF HOME HEALTH SUPERVISOR:
- Degree in Nursing from an approved school of nursing as determined by the Board of Nursing, required.
- Licensed to practice registered nursing in the state by the Board of Nursing.
- Recent experience in home health care setting.
- Understands principles of home health.
- Strong working knowledge of home health regulatory requirements.
- Strong computer skills.
- Strong leadership skills.
