North Country HealthCare is transforming healthcare for rural Arizona as a Primary Care, Federally Qualified Teaching Health Center. The coverage clinician must be a family medicine nurse practitioner with a minimum of 3 years experience. This is a hybrid in person and telehealth clinical position. The clinician is responsible for desktop coverage for terminated clinicians, clinicians out on FMLA or extended PTO. The coverage clinician will aid with population health care gaps during lulls in required desktop coverage and will assist in patient coverage during new clinician onboarding when there are gaps in insurance enrollment. In addition, the coverage clinician may be assigned special projects that impact organizational clinician workflow barriers as is appropriate. The coverage clinician is responsible for providing direct clinical care aimed at chronic disease management, preventative care, care gap closure, facilitating transitions of care, and contributing to overall efforts to improve overall population health and telehealth experience for North Country patients. The coverage clinician will not maintain an individual panel as they are responsible for closing gaps, connecting patients to care with newly established clinicians and short-term management while patients are being established to primary care team members panels.
This position will also contribute to quality improvement, especially as it pertains to continuity of care, decreasing patient and organizational risk and building appropriate empanelment.
