REGISTERED NURSE (RN) PART TIME - CLINICAL PRECEPTOR HOME HEALTH
LOCATION: CHICAGO NORTH SIDE TO WEST SUBURBAN CORRIDOR
HOURS: PART TIME DAYS M-F. NO WKDS/NO HOLIDAYS REQUIRED. MUST BE ABLE TO TRAVEL FOR STAFF ORIENTATION PURPOSES THRU NORTH CENTRAL Il CORRIDOR
Major Responsibilities:
- Coordinates orientation clinical experience with appropriate clinicians and departments to complete learning goals/competencies using adult learning principles to facilitate retention of essential information. Assesses/reassesses clinicians learning needs and collaborates with clinical excellence and leadership to tailor orientation to meet the individual needs and learning style. Fosters growth, learning, and enhances socialization of new team members.
- Delegates in accordance with State Boards, scope of practice and entity policy. Acts as professional role model for all staff following team member behaviors demonstrating ethical/legal/professional practice. Manages resources and patient flow through matching clinical competencies with individualized patient's and organization needs. Uses standards of care, entity policies/procedures and regulatory guidelines to guide practice.
- Collaborates with Clinical Excellence, leadership and quality in utilization of orientation modules, reference materials, competency testing tools, weekly goals and evaluations. Collaborates with Clinical Excellence through the provision of feedback to promote necessary changes to the program based on evaluations and effectiveness. Utilizes health outcome measures to improve performance and accountability for patient outcomes, patient experiences and safe delivery of care.
- Participates in direct patient care, organization-based quality improvement, evidence-based practice, and research activities as appropriate. Uses current healthcare research findings and other evidence to expand clinical knowledge, enhance role performance, and increase knowledge of professional issues. Acts as a role model to demonstrate patient experience through managing up and team member behaviors.
- Assesses patient's physical, psychosocial, cultural and spiritual needs through observation, interview, review of medical records & clinical data. Evaluates patient response and tailors' interventions to ensure optimal patient outcomes. Develops and implements patient plan of care and evaluates outcomes in accordance with current standards and in collaboration with the interdisciplinary team. Promotes, advocates and collaborates to protect the health and safety and rights of each patient. Maintain competency in all relevant skills and annual training requirements.
- Demonstrates competency in technology used in the post-acute setting. Compliant with HIPAA regulations through maintenance of patient confidentiality. Document ongoing status, interventions, patient response and outcomes in accurate, timely manner per agency expectations and policies. Uses information technology to communicate, manage knowledge, mitigate errors, and support decision-making.
- Seeks out learning opportunities, continuing education and experiences that reflect current practice in order to maintain skills and competence in clinical practice or role performance. Evaluates one's own clinical practice in relation to professional practice standards and guidelines, relevant statutes, rules and regulations. Demonstrates a commitment to the organization through ongoing participation at entity and unit-based meetings.
Licensure, Registration, and/or Certification Required:
- Registered Nurse license issued by the state in which the team member practices, and
- Basic Life Support (BLS) for Healthcare Providerscertification issued by the American Heart Association (AHA), and
- A valid drivers license issued by the Division of Motor Vehicles.
Education Required:
- Bachelor's Degree in Nursing.
Experience Required:
- Typically requires 3 years of experience in clinical home health or supervisory experience.
Knowledge, Skills & Abilities Required:
- Excellent communication skills.
- Interpersonal skills are essential to negotiate with patients, families, physicians, community agencies and third-party payers for an optimum plan of care.
- Basic proficiency in the use of Microsoft Office (Excel, PowerPoint and Word) or similar products.
- Analytical skills necessary to track outcomes.
Physical Requirements and Working Conditions:
- Position may require travel which may result in exposure to road and weather hazards.
- Operates all equipment necessary to perform the job.
COMPENSATION: $38.60-$57.90 per hour
