Healthcare Talent is assisting our client in their search for an Intake/Home Care Registered Nurse for their thriving offices in Garden Grove. The RN for Home Care is responsible for the development and implementation of home care services for program participants, including the coordination of all Durable Medical Equipment (DME) and community-based care. This position also works very closely with the Enrollment team to pre-screen potential participants and to provide additional community-based resources for potential participants who are not eligible or interested in the program.

Job Description
Position Responsibilities – Home Care
- Assess the home and personal care needs of the frail and elderly population, and identify and work with the Interdisciplinary Team (IDT) to develop specific plans of care. Initial and periodic assessments to be conducted minimally every 6 months and must be completed prior to the scheduled team meeting. Receives reports from home care staff and communicates participant changes to IDT members in a timely manner.
- Schedule and coordinate home care staff for in-home care delivery and the implementation of all personal care services to ensure that quality services are provided to meet participant needs.
- Coordinate all DME provisions in home setting; work with IDT with DME authorization requests.
- Document participant changes appropriately in the medical records and communicate participant changes to IDT in a timely manner.
- Supervise home care staff based in the community.
- Keep up-to-date on home care standards and best practices.
- Ability to drive to community locations and residences to provide on-site training and competency checks of home care staff.
- Other projects and duties as assigned.
Position Responsibilities – Intake
- Screen potential participants in the home and assess their environment and personal care needs of the frail and elderly population, and identify skilled needs based on DHCS eligibility guidelines.
- Complete LOC tool including diagnoses, medications, ADL’s, IADL’s in addition to a narrative summarizing potential participant’s skilled needs.
- Submitting completed LOC to DHCS.
- Tracking responses from DHCS and coordinating with Enrollment team.
- Presenting approved participants to IDT and sharing additional findings.
- If potential participant is deferred, coordinate with Enrollment team to bring potential participant into PACE for assessments.
- Track whether assessments have been completed and re-submit to DHCS for response.
- Other projects and duties as assigned.
Qualifications
Experience & Education
- Graduate of an accredited nursing school.
- Current RN license in good standing to practice in the State of California.
- Driver’s license and vehicle, or other approved means of transportation, and an acceptable driving record will be required for work away from the primary office 40% of the time or more.
- At least 1 year nursing experience with home or community health provider experience. Relevant experience must be within the last 3 years.
- At least 1 year experience working with the frail or elderly population.
