POSITION PURPOSE
Under supervision of a Director of Clinical Services, the Wound Care RN provides quality care to patients and performs technical skilled care in patient’s home in compliance with nursing standards, federal, state, local government regulations and company clinical policies.
ORGANIZATION
This position reports to the Director of Clinical Services.
ESSENTIAL FUNCTIONS
- Provides direct and individualized nursing care to assigned patients based on nursing standards and under the supervision of the Director of Clinical Services.
- Ensures quality and safe delivery of home health services to patients/clients and families/caregivers.
- Implements plan of care formulated by physician and Registered Nurse.
- Provides accurate and timely documentation consistent with plan of care.
- Assesses and provides patient/client and family caregiver education and information pertinent to diagnosis and plan of care.
- Maintains safe and healthy working environment and uses safe work methods and procedures in accordance with company clinical standards.
- Participates in coordination of home health services with Registered Nurse and clinical management.
- Assists patient in learning appropriate and executing self-care activities.
- Uses equipment and supplies effectively and efficiently.
- Documents and reports activity as required.
- Acts as an effective team member.
- Other duties as assigned.
KNOWLEDGE & EXPERIENCE
- Completion of state approved training program (generally in the form of a diploma, certificate, or associate degree) AND Associates Degree in Nursing or Bachelor of Science Degree for Nursing.
- Completed Wound Care Certification and 1 to 2 years of experience.
- RN license (pass NCLEX-PN exam).
- BLS certification.
- 1-2 years recent nursing experience.
- Medical knowledge, patience, compassion, kindness, critical thinking skills, knowledge of infection control practices, stamina, and physical endurance, observational skills, knowledge of occupational hazards and safety rules, communication skills, leadership, empathy.
MACHINES & EQUIPMENT
The incumbent must be able to effectively operate the current computer system, telephone system, and other office machines such as a copier.
