Location
Wentworth Health Partners Cardiology offers expert care in prevention, diagnosis, and treatment of heart disease to enhance your overall health. We are a full-service practice offering a full range of cardiology services including general and interventional cardiology, heart failure interventional procedures, electrophysiology, device implants, anticoagulation management and more.
This opportunity will support our Heart Failure Clinic, which is led by a Medical Program Director in coordination with APRNs. Our Nurse Practitioners specialize in heart failure and are able to adjust patient's medications and collaborate with cardiologists as needed. We also have a team of nurse navigators who guide our patients every step of the way - setting up clinic appointments and being the point of contact for patient visits and questions.
We are located at 19 Old Rollinsford Road in Dover, NH 03820. This site will be the predominant location for this position, but there may be occasional rotating coverage as needed in our other offices located at 67 Corporate Drive, Portsmouth, NH 03801 and 3 Terrascape Parkway, Somersworth, NH 03878.
The Opportunity
We are seeking a full-time, 40-hour Nurse Practitioner to support our Heart Failure Clinic team onsite Monday through Friday, 8:00am-4:30pm.
As true partners to our physicians, our Nurse Practitioners get the opportunity to work autonomously to the top of their education/certification while also functioning as part of the collaborative team.
The Nurse Practitioner is responsible for providing exceptional direct diagnostic and therapeutic patient care to the highest standards of the organization as part of a multidisciplinary team. The Nurse Practitioner participates in the delivery of patient care in the outpatient setting by performing initial and follow-up visits, as well as autonomous performance of approved clinical functions.
The Nurse Practitioner cultivates a patient centric environment which focuses on the whole individual inclusive of physical, psychosocial, spiritual and functional needs of the patient and family. The NP acts as a resource and collaborates with clinical staff and other hospital and health care disciplines for promotion of health to the optimal level of wellness for patients, families, and the community. This individual may provide autonomous as well as team-based care.

Qualifications

Education:
- Master's Degree in Nursing required
Experience:
- 2+ years of experience as a Nurse Practitioner required - with strong preference to those with prior Cardiology and/or Heart Failure experience
Certification:
- Certified New Hampshire Nurse Practitioner required upon hire
