How you make a difference
As a Care Management Specialist, you will report to the Director of Care Management and be responsible for completing various care management assignments. Your duties will include coordinating provider and clinic schedules, managing telehealth schedules, and utilizing our EMR for consultative services both on-site and off-site. To succeed in this role, you must be able to interrelate and work effectively with all levels of authority.
Key Responsibilities
- Manage incoming calls and messages, directing them to the appropriate staff.
- Schedule and track all assigned consultations and off-site referrals, ensuring timely processing.
- Input information into EMR and manage offsite requests as assigned.
- Maintain a filing system, making necessary updates and modifications to accommodate changes.
- Schedule appointments, reconcile monthly reports, and perform clerical responsibilities as needed.
Qualifications & Requirements
Education
• High school diploma or G.E.D.
Experience
• General knowledge of office practices and procedures. Computer skills with emphasis on spreadsheets and word processing. Formal secretarial training and medical terminology knowledge desirable.
