This position, under the supervision of the Residential Director, provides quality resident care as defined in Florida Code 10A-5 to support health goals and stability in an Assisted Living environment.
ESSENTIAL DUTIES AND RESPONSIBILITIES
- Completes assessments upon admission and monthly thereafter with each resident to maintain assisted living safely. Monitor clients’ progress providing referrals or interventions, as appropriate.
- Provide medication assistance including access, education and documentation.
- Assists residents with activities of daily living including supervision of diets.
- Provide limited nursing services as ordered as defined by limited nursing licensed (attached), if the service is not provided by home health and is required to maintain safe housing.
- Communicates with primary health care providers and pharmacies to ensure access and adherence to care.
- Maintains client record with health information and progress notes.
- Collaborate with Multidisciplinary staff to ensure comprehensive continuous care of residents.
- Maintains structure and order in the Residential Facility. Report and Document incidents, concerns and observations consistently.
- Engages clients in a strength-based supportive manner providing re-directions and guidance as necessary.
- Engages, counsels, manages, motivates, and effectively works with high-risk, vulnerable patient population, including chemically dependent persons.
- Engages, counsels, manages, and effectively works with persons in crisis.
- Provide supervision and support to support staff including milieu and care assistants.
- Attend and participate in staff meetings, program meetings, and staff training as determined by the Residential Director.
- Follows all policies and procedures both written and verbal.
SKILLS AND REQUIREMENTS
- Graduate of State accredited School of Licensed Practical Nursing and licensed to practice as an L.P.N. in the State of Florida and 3 years’ experience.
- Currently licensed in the State of Florida as a Licensed Practical Nurse.
- Experience or knowledge of Acquired Immune Deficiency Syndrome is preferred.

- Ability to effectively engage and work in a stressful environment with high-risk, vulnerable client population and persons in crisis.
- Ability to effectively communicate with people at all levels and from various backgrounds.
- Ability to maintain a strong sense of respect for confidentiality involving both clients and fellow employees.
- Ability to legally operate a motor vehicle and provide own transportation.
- Ability to work under pressure and to assess changing priorities and competing deadlines is essential.
- Flexibility and ability to cope with several tasks simultaneously.
- Must be physically and mentally fit to respond to emergency situations.
