As an Aging Care Manager 2, you will be responsible for interviewing and explaining the intake, assessment, and care management processes to our clients. Your work involves identifying and confirming needs and providing relevant accurate information and assistance to access the appropriate resources and programs to meet client needs. You will have the opportunity to meet with walk-ins and answer calls from the DHS/AAA SeniorLine Call Center. Other duties include responding to requests and messages within designated timelines, providing requested print information to be sent to participants, and completing and forwarding reports of need for consumer situations described as having the potential to meet protective services requirements.
Work Schedule and Additional Information:
- Full-time employment
- Work hours are 8:30 AM to 4:30 PM, Monday - Friday, with a 60-minute lunch.
- Salary: In some cases, the starting salary may be non-negotiable.

QUALIFICATIONS
Minimum Experience and Training Requirements:
- Six months as an Aging Care Manager 1 or a County Caseworker 1; or
- Successful completion of the County Social Casework Intern program; or
- Six months of professional human services or nursing experience; and a bachelor’s degree in social sciences, behavioral sciences, human services, or a closely related field; or
- An equivalent combination of experience and training.
