Job Description:
Position Overview:
We are seeking a dedicated and compassionate Social Worker to join our team in Seattle, WA. This hybrid role offers the opportunity to make a meaningful impact on the lives of our patients. The selected candidate will provide a wide range of clinical social work services in a primary care or home health setting, offering support and consultation to the care team in managing patients' needs. The primary goal is to evaluate psychosocial assets and deficits, enabling patients to maintain the greatest level of independence. Coordinating with other healthcare professionals, you will establish a comprehensive plan of care, deliver short-term counseling, provide education, and connect patients with external resources to meet their caregiving needs.
Key Responsibilities:
- Assess and evaluate the psychosocial needs of patients to create a comprehensive plan of care.
- Collaborate with the care team to ensure coordinated and holistic patient care.
- Provide short-term counseling to patients and their families.
- Offer education and resources to help patients maintain independence.
- Broker external resources and support services to meet caregiving needs.
- Contribute to the well-being of our patients by addressing their social and emotional concerns.
Qualifications:
- A minimum of one (1) year of previous social work experience is required.
- Per the National Agreement, current KP Coalition employees have the experience requirement waived.
- A Master's degree in social work (MSW) is necessary for this role.
- You must possess a valid license or certification as a Licensed Social Work Associate & Independent Clinical (Washington) OR a Licensed Advanced Social Worker License (Washington) OR a Licensed Independent Clinical Social Worker (Washington).
