Job Description Summary

The Assistant Director of Clinical Services is responsible for ensuring that patient care is coordinated and managed appropriately. The Assistant Director of Clinical Services is responsible for ensuring that care and services are delivered appropriately and for the supervision of clinical personnel.

Essential Job Functions/Responsibilities

Review case referrals, including home visits, to determine hospice/home care needs and assign appropriate hospice personnel.
Evaluate each case through home visits, conferences, and record review; guide clinicians to promote effective performance and quality home care services.
Review patient's medical diagnosis, prognosis, medications, procedures, and clinical course.
Assist clinicians in establishing therapeutic goals and developing plan of care.
Attend case conference meetings with hospice personnel to facilitate coordination of care.
Conduct quarterly record reviews and communicate findings to Clinical Director and hospice personnel.
Assist in the screening and interviewing process of new employees and orientation of new hires.
Assist Clinical Director in planning and evaluating inservice and continuing education programs.
Comply with accepted professional standards and principles.
Manage team assignments and schedules.
Approve time off requests for team members.
Review laboratory/x-ray equipment invoices for relatedness.
Monitor medications for effectiveness and compliance with pharmacy formulary.
Educate team on hospice policies and procedures.
Assess community health needs and assist in developing programs to meet those needs.
Participate actively in quality assessment performance improvement teams.
Perform other duties as delegated by the Clinical Director.

Qualifications

Registered nurse with current licensure to practice professional nursing in the state.
Graduate of NLN accredited school of nursing with a BSN degree preferred.
Previous experience in hospice/home care setting and two (2) years management or supervisory experience.
Complies with accepted professional standards and practice.
Excellent observation, good nursing judgment, and communication skills.
Understands hospice philosophy and issues of death/dying.
Understands principles of pain/symptom management.
Must be a licensed driver with an insured automobile in good working order.
Maintains current CPR certification.
Demonstrates excellent observation, verbal, and written communication skills.
