Job Title: Case Manager, Supportive Services

OVERVIEW: Founded in 1979 in direct response to rising rates of hunger and homelessness in North San Diego County, Interfaith Community Services works to break the cycle of poverty and homelessness by Helping People Help Themselves. Interfaith is the most comprehensive social services agency in North County, providing both immediate safety net services and long-term, life-changing programs designed to meet the holistic needs of people in crisis. WHAT YOU WILL DO: This Case Manager position will provide direct services to clients including, but not limited to, intakes, assessments, case management, and client advocacy (resources and referrals) to homeless and low-income individuals, with the goal of moving clients from crisis to self-sufficiency and supporting the mission of Interfaith Community Services.
- Provides direct services to clients including diversion, Individual Service Plan (ISP), assessment, crisis intervention, advocacy, social services, case management, and vocational case management.
- Assists participants in development of a strength-based/solution-focused, individualized goal and action plan that promotes permanent housing and self-sufficiency; develops an effective, timely referral network of services and support that assist with housing retention in order to ensure ongoing direction and support as needed.
- Utilizes Housing First and Diversion strategies to stabilize clients experiencing or at-risk of experiencing homelessness and works with participants in locating and securing housing; assists with housing applications and advocates for clients.
- Coordinates and provides services that are comprehensive, timely, effective, efficient, equitable, and client-centered.
- Identifies resources and advocates for clients’ needs.
- Links clients with resources within the agency and utilizes other public and private social service agencies and services as necessary, such as CalFresh, WIC, Social Security, and State Disability to increase household income.
- Develops financial goals with the client including developing and maintaining a household budget, becoming banked, paying rent and other bills on time, and increasing household income and credit score.
- Assists in the development of and encourages adherence to a personal budget through pro-active housing and budget counseling sessions; provides budget counseling and education to assist clients in establishing payment plans for bills and past debts and to assist clients in obtaining and maintaining their housing and self-sufficiency.
- Ability to work independently and professionally with partnering agencies, faith centers, and other community settings.
- Ability to work with and collaborate with volunteers, interns, and other agency personnel.
- Accurately records case information within electronic databases (HMIS/Clarity, CIE, ETO, and Excel) and client files, accurately completes all necessary forms, produces statistical reports, and utilizes the Coordinated Entry System.
- Participates in all staff meetings and trainings.
- Responsible for meeting specific outcomes required by program contracts.
- Performs additional duties as assigned.
EXPERIENCE / EDUCATION:
- HS Diploma or equivalent and 1+ year experience required.
- Bachelor’s Degree in Social Work or related field preferred or equivalent combination of education and experience preferred.
- Bilingual (English / Spanish) highly desirable.
- Experience working with formerly incarcerated individuals preferred.
