Job Summary:
The Providence Center Residential Case Manager - Brick Manor will provide clinical case management services to facilitate program goals and objectives using a quality care approach. Identify client needs with a person-centered approach to address mental health symptoms and co-occurring disorders, working with consumers to achieve the highest level of independence possible. Provide psycho-social rehab services to facilitate program goals in a quality care approach.
Schedule: Sunday, Wednesday & Thursday 3:00 pm - 11:00 pm, Saturday & Friday 7:00 pm - 3:00 pm.
Duties and Responsibilities:
- Provide case management and support to consumers.
- Make assessments and provide appropriate crisis interventions and risk management as needed to maintain a calm and safe environment.
- Teach consumers adult daily living skills assisting them in all aspects of their life in the group home and supervised apartments.
- Initiate treatment and advocate for consumers within the network of care.
- Negotiate on behalf of the consumer with landlords, courts, families, treatment team, consumers themselves.
- Assist in the development and implementation of socialization and recreational activities within the group home and community.
- Transport consumers in own vehicle or Center van to planned activities and appointments.
- Complete recordkeeping in accordance with Health Information Services (HIS) and other Center requirements.
- Maintain cooperative relationships with Center staff, consumers, community agencies, and the public.
- Attend trainings, case presentations, and conferences as required.
- Serve on appropriate Center committees.
- Work flexible hours, including evenings, weekends, holidays, and during inclement weather.
- Other duties may be assigned.
Requirements:
- Associate’s degree in Human Services required; Bachelor’s degree preferred.
- Experience in residential facilities helpful.
- Must have valid driver's license and insured auto.
- Ability to read and interpret instructions and follow procedures.
- Ability to write routine reports and correspondence.
- Ability to speak effectively with consumers, employees, and individuals in the community.
- Must be able to work flexible hours including evenings, overnights, weekends, and holidays.
