Registered Nurse
Under the direct supervision of the Nurse Manager/Nurse Supervisor and/or the Director of Nursing, the Registered Nurse is responsible for providing direct patient care to a diverse patient population across the continuum of care.
Essential Functions:
- Application of the Nursing process to assess, plan and carry out interventions in the management of patient's care in collaboration with the multidisciplinary care team including medical providers, mental health/behavioral health providers, case management, dental, optometry, registered dieticians, clinical pharmacists, health educators etc.
- Detailed, systematic documentation of all patient interactions in the electronic medical record.
- In-person and telephone triage and advice supported by recognized industry protocols; approved by the Chief Medical Officer and compliant with clinic policy.
- Nursing support for diverse population ranging from newborn to geriatric using culturally competent care.
- Coordination of post-hospital follow-up appointments for primary care patients.
- Nurse-led hypertension management through comprehensive One Community Health program.
- Results review, including lab and imaging results for issues ranging from chronic diseases to sexual health.
- Patient education and counseling for chronic disease management, medication adherence, preventive care, and wellness promotion.
- Medication administration as ordered by qualified providers; including PO, inhaled, IM, IV, and intradermal medications.
- Procedures and patient care as ordered, including venipuncture, wound care, port flushes, etc.
- Active support of health team members for the continuity of care provided to all patients at One Community Health.
- Patient scheduling of follow-ups and specialty appointments.
- Coordinating and follow-up with patient calls and EHR messaging within timelines set by clinic protocols.
- Leadership to clinical support staff including MAs and LVNs in a team-based model of care.
Minimum Requirements
- Associates in Nursing required
- 1 year clinical experience required
- Current active license to practice as an RN in the state of California.
- Active CPR, BLS Certification.
Preferred
- Bachelor's degree in nursing preferred
- 1 year acute care experience preferred.
- Experience with Epic EMR
- Demonstrated person-centered approach and familiarity with trauma informed systems and restorative practices.
