About this Role
Are you passionate about making a meaningful impact in the lives of the aging community by delivering personalized occupational therapy that enhances their independence and well-being? In this role, you'll have the opportunity to deliver personalized occupational therapy to PACE participants in various settings, including our care center, their homes, and even virtually. Your work will make a meaningful impact as you provide occupational therapy that meets each participant's unique needs, whether they require acute care, restorative support, or ongoing maintenance.
As a valued member of the interdisciplinary team (IDT), you'll collaborate closely to create and implement individualized care plans that truly make a difference in the lives of our participants. Partnering with our dedicated Physical Therapist, you'll also play a key role in ensuring the smooth operation of the rehabilitation department at our PACE center.

What will success look like as our Occupational Therapist?
- Collaborate and Contribute: Work closely with our IDT by completing assessments, care planning, and thoughtfully reviewing service requests to ensure the best outcomes for our participants.
- Empower Through Therapy: Provide compassionate occupational therapy to help maintain or improve each participant's functional abilities, while assessing their cognitive strengths to tailor personalized care.
- Support and Strengthen: Evaluate participants and recommend therapeutic exercises that promote physical well-being, adapting plans of care to meet individual needs. Engage participants and their caregivers in setting meaningful goals that empower them.
- Guided by Protocols: Deliver treatment with care and consistency, following established protocols to ensure each participant receives the highest quality of care.
- Educate and Enlighten: Share your knowledge with participants and their families, offering guidance on how to enhance mobility, safety, and independence, making a lasting impact on their daily lives.
- Ensure Home Safety: Conduct home safety evaluations with a focus on creating secure and comfortable environments for our prospective participants.
- Support Transitions: Collaborate with the IDT to ensure smooth care transitions between external settings and home, making each participant feel supported and cared for.
- Manage Equipment Needs: Assist with the ordering, inventory, distribution, and maintenance of durable medical equipment, ensuring our participants have what they need to thrive.
- Partner with Care Facilities: Work hand-in-hand with nursing home, skilled nursing facility, and assisted living staff to address the rehabilitation needs of our participants, advocating for their well-being.
- Lead with Approval: Offer your expertise in approving care plans on behalf of the IDT, ensuring our participants receive consistent and thoughtful care.
- Home Visits: Complete home assessments and deliver treatments in the comfort of our participants’ homes, bringing care directly to where it’s needed most.
- Support in overall growth: Additional support in duties and responsibilities to provide support for growth in center and organization.
What does an ideal candidate look like?
We're looking for individuals who bring passion and a strong sense of mission to our team, particularly in serving high-risk seniors and frail older adults. If you’re someone who listens deeply, communicates clearly, and enjoys working as part of a team, we’d love to hear from you!
- Educational Background: A graduate of an accredited Occupational Therapy program.
- Professional Certification: Occupational Therapy registration from the National Board of Certification.
- Essential Certifications: CPR and First Aid certification (required within the first 60 days of employment).
- Licensing: A valid California Occupational Therapy License, or proof of eligibility to obtain one.
- Experience: Ideally, two (2) years of experience in Occupational Therapy, especially in a clinical setting with frail or elderly populations. A strong preference in PACE programming would be ideal, but not required.
- Driving Requirements: A valid CA driver’s license, personal transportation, a good driving record, and auto insurance as required by State law. Position will require from time to time to visit participants in-home for assessments.
- Physical Ability: Ability to lift, carry, or otherwise move up to 50 pounds as needed.
- Participant Support: Comfort in assisting participants with mobility challenges, ensuring their safety and well-being.

- Schedule is Monday - Friday 8a-5p.
- Free, secure, and private parking available on-site.
Compensation:
$100,400 - $122,300 a year

Base pay is based on several factors including but not limited to education, relevant work and industry experience, certifications, and location of the role. Onsite roles include appropriate geographic adjustments, while remote roles are typically priced off national pay data.
