Physical Therapist (PT) Per Diem
We are seeking a dedicated Physical Therapist to join our dynamic team. You will assess and treat patients with a variety of needs, part of an interdisciplinary team focused on physical therapy. Essential Duties and Responsibilities:
- Review available patient information to evaluate the impact on the assessment and treatment process.
- Conduct and document thorough functional assessments of patients, providing a clear and detailed treatment plan with achievable functional goals.
- Comply with all evaluation, treatment, and documentation guidelines, maintaining timely and accurate records.
- Instruct and provide close supervision to Assistants involved in patient treatments.
- Enter progress notes into patient records following each treatment session and revise treatment plans as necessary.
- Provide a clear, concise discharge summary, documenting the course of treatment.
- Work collaboratively with nursing staff, patients, and families to maximize patients' functional abilities.
- Attend and participate in rehabilitation team conferences to discuss patient progress, problems, or needs.
- Orient patients to program services, inform them of procedural risks, and provide therapeutic instructions for home care.
- Access continuing education opportunities to maintain clinical competency, and participate in quality improvement processes.
- Adhere to all professional standards and guidelines, maintaining productive interaction within the community to promote the program.
Minimum Position Qualifications:
- Bachelor/Master of Science degree in Physical Therapy.
- Current licensure in the state of practice.
- Superb oral and written communication and interpersonal skills.
- Demonstrated competency in physical therapy principles, methods, materials, and equipment.
- Ability to develop and implement comprehensive care plans based on patient needs.
- Familiarity with accreditation standards and compliance requirements.
Preferred Qualifications:
- Experience in supervising and training assistants in a clinical setting.
- Certifications and demonstrated competency in specialty areas of physical therapy.
- Comprehensive knowledge of OBRA regulations, HIPAA, and third-party reimbursement guidelines.
Skills:
- Excellent assessment and documentation skills.
- Strong ability to communicate effectively with patients, families, and other healthcare professionals.
- Deep understanding of therapeutic and rehabilitative processes.
- Proficiency in using physical therapy tools and equipment.
