Responsibilities
- Assess patient status by obtaining health history through patient/family interviews and chart review; assess presenting illness, risk factors, and family history, psychosocial situation, and cultural factors; and perform appropriate physical examination.
- Order/perform appropriate laboratory diagnostic and other screening tests.
- Seek other information as needed, including consultation with other providers and other disciplines, for evaluation of illness.
- Develop and implement treatment plan by prescribing/dispensing medications and/or injections in compliance with medical practice guidelines and state laws.
- Instruct patient/family regarding medications and treatments.
- Educate patients regarding health promotion/illness prevention.
- Recommend appropriate community resources to meet patient/family needs.
- Communicate appropriate case management information to other professionals and community agencies.
- Prepare documentation for medical records including updating patient medical chart by posting examination and test results, diagnosis, medications, and treatment in written/computerized manner.
- Participate in peer reviews, chart reviews, staff education, clinical guideline development, and other continuing education and quality assurance activities to demonstrate compliance with standards, regulation, policies, and procedures.
- Collaborate with physicians when indicated in managing acute and long-term medical needs of patient.
- Provide monitoring and continuity of care between visits according to treatment plan including triaging patient calls/e-mails.
- Participate in professional development activities and maintain professional affiliations.
Department/Role Specific Responsibilities
- Cheyenne Family Medicine: Participates in Patient Centered Medical Home Care Team collaboration.
- Children’s Clinic: Participates in Patient Centered Medical Home Care Team collaboration.
- Family First: Participates in Patient Centered Medical Home Care Team collaboration.
- Hospitalist Service: Inpatient care. Admissions and cross coverage of hospitalized adult patients.
- Palliative Care/Hospice: Assist patients with chronic and/or terminal illness in determining goals of care and providing symptom management. On Call Services with provider rotation. Attend weekly interdisciplinary meetings.
- Plaza Family Medicine: On call telephone services for Primary Care to rotate with other providers. Manage Chronic and Acute illness in the outpatient Primary Care Setting. Participates in Patient Centered Medical Home Care Team collaboration.
Desired Skills
- Knowledge of advanced practice of nursing including anatomy and physiology, pathophysiology, pharmacology and therapeutics, basic nutrition, behavioral factors, psychosocial/family systems, diagnostic testing, interpretation of results and clinical decision making, alternative treatment modalities.
- Knowledge of professional nursing theory, practices and regulations related to evaluating and providing patient care, and professional ethics related to the delivery of nursing care.
- Knowledge of patient assessment techniques including taking medical histories, compliance with care recommendations.
- Ability to diagnose and treat complex medical problems, analyze data and develop a plan of care utilizing evidence-based medicine.
- Skill in gathering and analyzing physiological, socioeconomic, and emotional patient data.
- Knowledge of health promotion, health risk identification, and patient education principles and techniques.
- Knowledge of community, professional, and educational resources.
- Skill in case management, time management, problem solving and crisis intervention.
- Ability to establish/maintain effective working relationships with patients, clinical staff, and the public.
- Excellent verbal, written and interpersonal communication skills.
Job Requirements
Here’s what you will need:
- Completion of an accredited Acute Care Nurse Practitioner/Advanced Practice Nurse program.
- Nurse Practitioner/Advanced Practice Nurse licensure in the State of Wyoming.
- Cheyenne Regional RQI within 14 calendar days of start date.
- Specialty certifications as required per department/clinic specifications.
- Controlled Substance Licensure from the Wyoming State Board of Pharmacy.
- Federal DEA (Drug Enforcement Administration) licensure.
