Job Role Summary
Performs nursing services as a health team member in Pediatric, Adolescent, OB/GYN, Family Planning, and/or School-Based Clinics. Provides Health Education and patient counseling to control and prevent disease, and promote optimum health and well-being of clients, adhering to all laws, regulations, and policies and procedures.
Associated Job Duties
Responsibility: Collaborates in the provision of patient and family care
Develop effective working relationships with patients, families, and health professionals in a multi-disciplinary team.
Assists physicians, nurse practitioners, and other team members with diagnostic procedures and regimens; participates as a team member in clinics.
Works collaboratively with physicians, nurse practitioners, and other health professionals to implement treatments, dispense, and administer medications and injections in accordance with prescribed orders and instructions.
Works as a member of a health team to counsel patients and their families in health education, medication, and treatment instructions; provides disease prevention guidelines and surveillance of health practices with regard to physiological and social processes.
Develops and implements nursing care plans based on individual needs and makes appropriate changes based on responses of the patient/family; initiates patient referrals to appropriate agencies/organizations to promote continuity of care for patients and families. Provides case management and intensive follow-up and assures the patient is aware of the need for services and the care plan.
Responsibility: Maintains appropriate medical clinic environment and patient flow
Responds to health needs of patients and their families when seen in the clinics (ACTION Health Center or School-Based Health Clinics) by identifying and assessing health care requirements or problems expressed. Phone triage, assessment, and prioritization of patient needs.
Directs and coordinates patient flow.
Reviews/monitors/guides health care activities of appropriate personnel.
Informs appropriate personnel of the need for clinic and pharmacy supplies when needed (ACTION Health Center and School-Based Health Clinic).

Responsibility: Documentation and EMR Management
Interviews patients and records health history, prepares, maintains, and uses records and reports for direct patient/client services.
Documents routine nursing activities and maintains records; prepares charts, collects statistical data, and prepares reports.
Has access to confidential patient health information (PHI) and may have the need to transport PHI off-site.
Responsibility: Professional Development and Public Health Outreach
Assumes responsibility for professional self-growth and continued development in nursing.
Responsible for adherence to all laws, regulations, policies, and procedures including HIPAA, OSHA, CLIA, and FERPA.
Uses professional judgment; accepts responsibilities for decisions in planning/implementation/interpretation.
Assists with generation, evaluation, and implementation of clinical paperwork, education materials, and handouts.
Participates in audit educational programs and development of educational materials.
Participates in Public Health emergency preparedness exercises and in the response to public health emergencies as directed.
Participates in a minimum of three (3) H.O.P.E. events totaling a minimum of twelve (12) hours annually.
Qualifications
- Completion of a Bachelor of Science in Nursing
- Graduation from a NLN approved program for registered nurses
- Current unrestricted license as a Registered Nurse
- Previous clinical experience as a professional registered nurse is preferred
