Nurse Navigator - Oncology Breast Center Lansing
Provides direct patient care and comprehensive coordination of care for patients and their families/caregivers. Eliminates barriers to timely care, facilitates flow through the system, increases patient and provider satisfaction, maintains point of contact with providers, and serves as the first point of contact for patients and families. Provides a full range of nursing services, including assessment, treatment planning, and therapeutic interventions in an interdisciplinary environment consistent with professional practices and ethical standards.
Responsibilities include:
- Providing direct patient nursing care for specified patients.
- Performing nursing assessments and triage of patient care needs for new and ongoing patients.
- Overseeing the planning, scheduling, and implementation of day-to-day clinical activities and procedures for navigated patients.
- Acting as the central point of contact for all navigated patients, including interaction with medical, nursing, and ancillary services.
- Providing education to patients, families, and significant others; acting as an information resource to students, healthcare professionals, patients, and the public.
- Collecting and reporting data from patients on barriers, healthcare, medications, prior treatments, etc.
- Tracking order completion and follow-up; monitoring patient compliance with treatment plans.
- Facilitating all stages of the referral process to outside resources and care coordination as needed.
- Designing or participating in the creation of health education materials.
- Providing and documenting quality, personalized, and evidence-based care and interventions.
- Maintaining professional and technical knowledge by attending educational workshops, reviewing professional publications, establishing personal networks, and participating in professional societies.
- Complying with federal, state, and local legal and certification requirements, including HIPAA and OSHA.
- Performing other related duties as required and directed.
Required:
- Nursing degree from an accredited educational institution.
- State licensure as a registered nurse.
Preferred:
- Bachelor's degree in nursing.
- One year prior work experience in a clinical area.
