Job Summary:
The Providence Center Residential Case Manager Ledge Street Apartments will provide clinical case management services in order to facilitate program goals and objectives using a quality care approach. Identify client needs with a person-centered approach to address not only mental health symptoms but co-occurring disorders. Working with consumers to achieve the highest level of independence possible.
Schedule: Tuesday first shift 7am-3pm, Wednesday first shift 7am-3pm, Thursday second shift 3pm-11pm, Friday first shift 7am-3pm, Saturday first shift 7am-3pm.
Duties and Responsibilities:
- Initiate treatment and advocate for consumer within network of care.
- Assist in the development and implementation of socialization and recreational activities within the group home, in the community, and by attending Day Program groups and activities.
- Complete recordkeeping in accordance with Health Information Services (HIS) and other Center requirements and abide by Center confidentiality policy.
- Provide case management and support to clients.
- Make assessments and provide appropriate crisis interventions and risk management as needed to maintain a calm and safe environment.
- Teach clients adult daily living skills assisting them in all aspects of their life in the group home and supervised apartments.
- Ability to read and interpret instructions documents and follow procedures.
- Ability to write routine reports and correspondence.
- Ability to speak effectively with clients, employees, and individuals in the community.
- Maintain cooperative relationships with Center staff, clients, community agencies, and the public.
- Negotiate on behalf of the client, as needed, with landlords, courts, families, treatment team, clients themselves.
- Transport clients in own vehicle or Center van to planned activities and appointments.
- Attend trainings, case presentations, and conferences as required.
- Other duties may be assigned.
- Work flexible hours, including evenings, weekends, holidays, and during inclement weather when unforeseen circumstances arise to ensure 24-hour coverage.
Requirements:
- High School Diploma/GED required. College degree preferred. If no college degree, must have 5 years of relevant experience.
- Experience in residential facilities is preferred.
- Must have valid driver's license and insured auto.
