If yes, how you will contribute to the team:
As a Licensed Vocational Nurse / Licensed Practical Nurse (LVN/LPN), you will put resident service first and ensure that residents and their families receive the highest quality care. Under the direction of a registered nurse, you will deliver a full range of nursing care. This includes but is not limited to:
- Provide skin and wound care and treatments, as ordered.
- Identify, manage, and treat primary and secondary wounds, including pressure ulcers/injuries, ostomies, surgical wounds, traumatic wounds, and burns.
- Help identify and mitigate risk factors for secondary wound infections.
- Help identify and mitigate specific resident risk factors for pressure ulcers/injuries.
- Assist the interdisciplinary team in identifying and managing resident pain related to wounds.
- Complete regular skin assessments on residents and document the findings.
- Admitting, transferring, and discharging residents.
- Conducting daily rounds to observe the resident’s physical and emotional status.
- Medication administration.
- Monitoring resident care provided by unlicensed staff.
- Maintaining the confidentiality of all resident care information.
- Promoting a culture of safety to ensure a healthy practice and living environment.
Must Have:
- LVN / LPN with current licensure to practice in the state.
- Strong communication and organizational skills.
- Basic knowledge of computer use or willingness to learn.
Nice to Have:
- Experience in Skilled Nursing Facility (SNF) environment.
- Nursing experience providing direct care to multiple residents.
- Experience with Point Click Care (PCC).
