In this vital field role, you will provide direct patient care and manage therapy cases within the community to optimize healthcare outcomes. Responsibilities
- Provide physical therapy services in patients homes.
- Perform assessments, develop personalized treatment plans, and deliver therapeutic interventions.
- Educate patients and families on home exercise programs and techniques.
- Monitor and document patient progress, adjusting treatment plans as needed.
- Collaborate with healthcare professionals to ensure comprehensive care.
- Design and/or order appropriate adaptive/assistive devices.
Requirements
- License: Active Physical Therapist (PT) license in Arizona with no restrictions
- Experience: At least 1 year of experience supporting in-home patients
Skills:
- Proficient in therapy software and documentation
- Strong organizational and communication skills
