Essential Functions:
Responsible for advanced practice in physical and psychosocial assessment, diagnosis and management of health and illness needs. Performing in-clinic procedures at our ambulatory-based locations, the NP will also collaborate with and support other providers and caregivers in the wide network to effectively improve the overall health of our communities. Assumes responsibility and accountability for the continuity of health care and health education regardless of the presence or absence of disease. Utilizes standardized procedures and consults their Supervising Physician, the CMO and/or Medical Services Manager in cases out of their scope of practice or out of program scope. Nurse Practitioner will document, via EMR, all treatment protocols and interventions utilized in a timely manner. Services are evidenced-based and tailored to patient population needs. All services will be performed confidentially, in accordance with local, state, and federal laws. Will participate in case consultations and will support overall continuity of care for clinic patients, as needed and guided by department and patient needs. Adheres to department and organizational programs. Upholds all HIPAA and other regulatory compliance and safety requirements. Stays current on all credentialing requirements.

Ability to write treatment notes and encounters. Ability to speak effectively to all team members and levels within the organization.
Reasoning & Mathematical Skills: Ability to add, subtract, multiply and divide in all units of measure, using whole numbers, common fractions and decimals. Ability to apply common sense understanding to carry out instructions furnished in written, oral or diagram form. Ability to deal with problems involving several concrete variables in standardized situations.
Physical Demands: Repetitive motions and routine use of standard office equipment such as medical devices, computers, telephones, copiers/scanners, filing cabinets. Ability to see, speak, walk, hear, stand, use of hand/fingers to handle or feel; climb stairs, stoop, carry/lifting up to 20 lbs. Prolonged periods of sitting at a desk and working on a computer. Frequent standing, and movement up and down in chairs, in and out of patient examination rooms. Ability to physically conduct all assessment and treatment measures of patients. Specific vision abilities required by this job include close vision, color vision, peripheral vision, depth perception and the ability to adjust focus. The noise level in the work environment is usually mild. It includes open offices, office equipment, such as, phones, computers, and printer/copy machines, and waiting areas. The demands of the position include travel to assigned clinics and occasional organization-wide activities.
Requirements
Required Qualifications:
Education:
Master’s Degree from an accredited Nurse Practitioner program.
License/Certifications:
Certification from an accredited Nurse Practitioner program.
Valid Nurse Practitioner furnishing number issued by the California Board of Registered Nurses.
Valid California Registered Nurse/ Nurse Practitioner license.
Provider level basic life support (BLS) from AHA approved program.
DEA Certification.
Experience:
Minimum of 6 months in an ambulatory or community setting.
Knowledge/Skills/Abilities:
Proficiency in EMR Systems.
Demonstrated ability to read, write and speak English fluently.
Ability to utilize intermediate/medical Spanish.
Proficient in computer programs and systems, including email, internet, and Microsoft Office suite.
Knowledgeable in applying evidenced-based medicine to treat and diagnose. Demonstrated ability in applying methods of scientific inquiry and exercising clinical judgment in challenging situations.
Commitment to continuous learning and professional development, remaining current on medical practices and professional credentialing and licensure necessary for continuous employment with St. Jude Neighborhood Health Centers.

Experience:
Experience providing direct patient care, in a Federally Qualified Health Center (FQHC).
Knowledge/Skills/Abilities:
Proficient in Epic EMR Systems.
Salary is based on a 40-hour work week.
Salary Range is $138,500 to $155,102. DOE
