RN CASE MANAGER RESPONSIBILITIES:
- Conducts a thorough and accurate initial evaluation, with additional patient and personnel assessments as required, within appropriate time frames and determines the patients clinical and social needs.
- Identifies, develops, implements, updates and evaluates a plan of care for each patient and coordinates the patients care with the patients physician assuring that patient needs are continually assessed.
- Demonstrates knowledge and compliance with CMHHA Policies and Procedures as well as federal, state and local rules and regulations.
- Demonstrates leadership skills in coordinating team and patient care and referrals by effective planning, efficient time and supply management, supervision and delegation of LPN and Aide duties and feedback to team members and the Director.
- Maintains desired level of productivity for all duties, exercising sound judgment in decision making, following through on all projects and demonstrating thoroughness and accuracy.
- Demonstrates professional responsibility in the Case Manager role by functioning as a role model and identifying opportunities for self-development and enhancement of professional competency.
- Contributes to a positive work climate and the promotion of team collaboration.
