There is an immediate opening for a Home Health Palliative Care RN to take their rapidly growing visiting nurses program to the next level.
The selected candidate will perform nursing services requiring the use of physical, social sciences, and nursing skills in the care of the sick, the conservation of health, and the prevention of disease. Responsible for the care and case management of the patients assigned. Develops a care plan that is patient-centered and establishes goals, based on nursing problems and incorporates palliative nursing actions and integrated care management principles.
Education:
- Graduate of a state approved school of nursing; Bachelor's degree preferred
Work Experience:
- Minimum one year relevant experience.
License/Certification:
- Current RN licensure by the state of WI or eligible
- Current CPR certification; Requires a valid driver's license (for a minimum of 12 months), a satisfactory driving record, and proof of automobile insurance equal or greater to that required by the state of Illinois.
- Certification in Hospice/Palliative Care or willing to obtain.
- RN license preferred.
