NFI Massachusetts Inc., a nonprofit human services organization, is seeking a dedicated and organized Case Manager to join our residential youth program in Haverhill, MA.
As a Case Manager, you will play a key role in supporting our clinical team by coordinating critical aspects of care for our residents. From scheduling treatment team meetings and liaising with school districts, SEIS, and family members, to working closely with DMH/DCF caseworkers, you’ll be instrumental in ensuring seamless case coordination and communication. Your responsibilities will also include maintaining up-to-date documentation, data collection, and occasionally supporting residential staff in the daily activities of the program when needed.

Job Responsibilities:
- Coordinate treatment planning meetings for clients from pre-admission to post discharge.
- Support clinical and management staff with administrative tasks as assigned.
- Participate in day-to-day program structure including groups/activities when possible.
- Attend and provide documentation for weekly TEAM meetings.
- Attend treatment team meetings and associated documentation.
- Participate in community meetings.
- Coordinate with Transitional Specialist around tasks associated with discharge planning.
- Available for crisis intervention and support in the milieu.
- Perform physical intervention in the event of a youth crisis.
- Perform all duties in accordance with the Agency’s policies and procedures.
- Support the administrative team with 24 hour on call coverage.
Job Requirements:
- Experience with youth population required.
- Bachelor’s degree in related field required.
- High energy level, superior interpersonal skills and ability to function in a team atmosphere.
- Must have excellent organizational skills.
- Strong communication skills.
- Must hold a valid driver's license.
Salary: $54,000
