Job Description - CRSW Case Manager - The Doorway (2400525)
Who You Are:
As a Certified Recovery Support Worker (CRSW) Case Manager for The Doorway at Catholic Medical Center (CMC), you'll play a vital role in assisting patients on their journey to sustained recovery from substance use disorders. You understand the importance of empathy, confidentiality, and respect in providing care to vulnerable populations.
This position is a fully benefitted full time (40 hour) position, Monday through Friday.
What You'll Do:
- Engage with patients referred to The Doorway program from various sources, including hospitals, healthcare providers, and community agencies.
- Conduct intake, follow-up, and discharge interviews to assess patient needs and enroll them in the Doorway program.
- Collaborate with The Doorway team to plan strategies for communication, referrals, and transitions of care.
- Assist patients in accessing healthcare services and community resources, supporting them in achieving their recovery goals.
- Document all patient interactions accurately and confidentially in electronic medical records.
- Participate in team meetings and educational initiatives to promote overdose awareness and naloxone administration in the community.
- Maintain positive relationships with team members and community partners, adhering to professional ethics and boundaries at all times.
What You'll Bring:
- High school diploma required - Recovery Support Worker Certification must be obtained within 9 months of start date.
- Exceptional interpersonal skills and the ability to work effectively with diverse groups.
- Critical thinking abilities and a proactive approach to problem-solving.
- Strong verbal communication skills to convey important instructions and information.
- Willingness to work in a fast-paced environment with frequent interruptions and adapt to unexpected situations.
- Basic computer skills and the ability to learn new software systems.
- Experience or willingness to work with vulnerable populations, and understanding of mental illness and substance use disorders.
