Provide Diabetes Self-Management Training & Education, Medical Nutrition Therapy, and Oncology Nutrition for adult and pediatric patients. Requires travel to outlying clinics to provide nutrition and diabetes services. This position requires CDCES or BC-ADM certification within 12 months of hire
Position Reports To: Director Nutrition Therapy & Diabetes
Essential Functions
- Perform diabetes self-management education; including diet, exercise, self-monitoring of blood glucose, medication adherence and insulin injection training.
- Participate in and run nutrition and diabetes education classes; responsibility to instruct classes, facilitate the knowledge, skill and ability necessary for nutrition, prediabetes and diabetes self-care.
- Demonstrate understanding of the stages of change in relation to behavior modification and nutrition.
- Provides Glucometer, CGM, insulin injection and insulin pump training, acquiring pump certification as needed.
- Develop community based nutrition and diabetes resources including Teleheath.
- Provide Medical Nutrition Therapy to multiple patient populations of all ages, including Oncology patients.
- Keep abreast of current national Diabetes initiatives and activities, developing a network of resources and contacts.
- Chart notes completed per organization policy.
- Ability to function independently and as a member of a cross-functional team.
- Participates in LEAN performance improvements and research activities.
- Required to maintain active Basic Life Support (CPR) certification.
- Other duties as assigned.

Qualifications
Required:
- Bachelor’s degree in Food and Nutrition or related field.
- CDCES or BC-ADM certification within 12 months of hire.
- Registered with the Commission on Dietetic Registration.
- Washington State Certified Dietitian (CD) - (may apply upon hire)
- Required to maintain active Basic Life Support (CPR) certification and Washington State Food Handlers Permit.
- WA DOH Medical Assistant Registered License (apply after hire).
- Must be proficient with Windows navigation, Microsoft Word, Excel and Outlook.
Desired:
- Master’s degree.
- Academy of Nutrition and Dietetics or ASPEN registration.
- Software/Platform Skills: Electronic Medical Records experience.
