Job Details
Responsibilities:
- Evaluating patients' physical condition, functional capabilities, needs, and goals to formulate effective treatment plans.
- Implementing individualized treatment programs designed to restore, reinforce, and enhance performance.
- Identifying and documenting goals, anticipated progress, and plans for reevaluation.
- Providing education to patients and families regarding physical therapy treatment plans and promoting wellness and injury prevention.
- Collaborating with a multidisciplinary team, including doctors, nurses, occupational therapists, and speech therapists to ensure a comprehensive approach to patient care.
- Maintaining accurate, detailed reports and records, tracking patients' progress and adjusting treatment plans as necessary.
- Participating in professional development activities and staying updated with the latest in physical therapy techniques and technology.
Qualifications:
- Doctor of Physical Therapy required.
- A valid Physical Therapist license in the state of practice.
- A degree in Physical Therapy from an accredited program.
- Excellent communication and interpersonal skills, with a patient-centric approach to care.
- Proficiency in the use of physical therapy equipment and technology.
- Ability to work both independently and as part of a multidisciplinary team.
- Strong problem-solving skills, with the ability to adapt treatment plans to individual patient needs.
- Ability to manage a diverse caseload, prioritizing tasks effectively.
- A valid driver's license and reliable transportation for home visits as required.
