Position Summary: The Registered Nurse (RN) Public Health Case Manager oversees comprehensive case management for individuals requiring public health care such as well-child checkups, immunizations, tuberculosis screening, and management of chronic health conditions. The role entails training Community Health Aides and Practitioners (CHA/Ps) to competently administer public health testing, examinations, and immunizations.
Responsibilities:
- Participating in the design, execution, monitoring, quality assurance, and assessment of the Well Child/Immunization and other public health programs.
- Identifying areas of improvement and proposing effective strategies to enhance program performance.
- Traveling to various villages to render services and provide on-site instruction.
- Supporting CHA/Ps in conducting examinations, assisting with testing, and administering immunizations.
- Providing education on topics related to immunizations, chronic care, preventive health examinations, and diagnostic and surveillance testing.
- Assisting in providing instruction during Health Aide Training and at Continuing Medical Education (CME) events.
- Coordinating with other health care providers and maintaining accurate records of patient interactions.
- Monitoring patient progress and ensuring patients' health needs are met effectively.
- Staying updated with the latest public health knowledge and developments.
Position Qualifications:
Minimum Education: Diploma or AAS for RN, prefer BSN.
Minimum Experience: 2 years of positive work history as RN.
License, Certification, Registration: Must have current licensure in State of Alaska. Current BLS, recommend ACLS and PALS.
Equipment/Tools: Must have computer experience. Proficiency in MS Office software including WORD, EXCEL and ACCESS preferred. Must be adept with RPMS and its modules within 3 months of hire. Must be proficient with equipment and tools used in an EPSDT exam within 3 months of hire.
Specialized Knowledge and Skills: Knowledge and experience with Pediatrics required. Prefer experience in teaching, working in cross-cultural setting and knowledge of the Community Health Aide Program. Excellent verbal and written communication skills. Fluent in English.
