Overview:
A leading home health care agency based in Indianapolis, IN is seeking a qualified Clinical Director to oversee and manage all aspects of clinical care. This leadership role requires deep knowledge of home health services, Medicare and Medicaid regulations, and a passion for delivering top-notch patient care. As a Clinical Director, you will ensure compliance with state and federal regulations, supervise nursing staff, and contribute to the agency's quality improvement efforts.

Responsibilities:
- Provides comprehensive assessments (initial and ongoing) including OASIS items for Medicare and Medicaid patients.
- Develops and implements the patient's plan of care in accordance with the patient's primary care provider.
- Reviews the patient's medications and updates the patient's medication profile when there are new, changed, or discontinued medications.
- Evaluates the patient or caregiver/family members ability to manage medications appropriately.
- Assigns and supervises LPNs and Home Health Aides as required by Medicare/Medicaid and state regulations, if any.
- Participates in orientation and training of new personnel.
- Evaluates and completes competencies for Registered Nurses, LPNs, and Home Health Aides as directed by the Clinical Manager.
- Maintains accurate and timely documentation of care/services provided.
- Notifies the primary care provider, as applicable, when there are changes in the patient's plan of care or clinical changes.
- Communicates with other team members, as appropriate, regarding the patient's plan of care, and updates team members when there are changes.
- Attends and participates in staff meetings, educational in-services, and committee meetings, as appropriate.
- Participates in the on-call rotation to ensure the agency provides 24-hour nursing services.
- Reports adverse events, incidents, and grievances/complaints (patients and/or personnel) to Supervisor.
- Provides education and information to the patient and/or family members as appropriate to their plan of care.
- Evaluates the patient's progress and effectiveness in meeting goals and outcomes.
- Participates and coordinates the patient's discharge from service.
- Participates in the agency's Quality Assessment and Performance Improvement (QAPI) program, as appropriate.
- Adheres to the agency's policies and procedures.
- Maintains confidentiality of all aspects of job performance.
- Practices proper infection control and utilizes PPE as indicated.
- Only performs treatments or special procedures as authorized by the Home Health Agency and deemed competent to perform by the Clinical Manager.
- Complies with all applicable federal, state, and local laws and regulations in performance of job functions.
- Performs other related functions as directed.
Qualifications:
- Must have a valid Registered Nurse (RN) license in the state of Indiana.
- Minimum of 2 years of clinical experience, with at least 1 year in a leadership or management role within home health care or other healthcare setting.
- Strong understanding of Medicare/Medicaid regulations, OASIS training preferred.
- Exceptional leadership, organizational, and communication skills.
- Experience in home health care staff management and development.
- Ability to handle the fast-paced environment of home health care services.
