Speech Language Pathologist (SLP) Career Opportunity:
We are seeking a Speech Language Pathologist (SLP) to join our team on a part-time or PRN basis. The SLP will provide in-home speech therapy services for residents in senior living communities as well as individual homes throughout Marion and Hamilton counties. This position will serve residents in the following BHI Communities:
- Hoosier Village Retirement Community is a stunning 300-acre Life Plan Community located near the quaint town of Zionsville, Indiana.
- The Barrington of Carmel is an upscale Life Plan Community located in the heart of downtown Carmel, Indiana.
- Westminster Village North is set on 57 beautiful and historic acres located on the northeast side of Indianapolis, just south of Geist Reservoir.

Key Responsibilities:
- Provide Speech Therapy to patients in line with individual client guidelines and CPHC standards.
- Screen and evaluate patients to determine the need for intervention, and develop comprehensive treatment plans with clear goals.
- Communicate evaluation findings and treatment plans to patients, families, and other team members, coordinating schedules and care.
- Implement and adjust treatment plans based on patient response, adapting methods to meet individual needs and physician orders.
- Develop home or community programs to support patients' progress in their own environments.
- Maintain accurate documentation of assessments, treatments, progress, and discharge plans.
Requirements:
- Must hold active Indiana Speech Language Pathologist license and Certificate of Clinical Competence.
- CPR certification (ACLS preferred).
- One year of experience in home health setting required.
