Division/Program Description
Felton’s Senior Division specializes in a full continuum of mental health services for older adults in outpatient clinics, the community, and supportive housing sites, as well as programs in employment training, economic security, case management, advocacy, and social connection and inclusion. The Intensive Case Manager (ICM) and Geriatric Outpatient Services (GOS) are both part of Felton/Family Service Agency’s Geriatric Mental Health Model, which offers mental health services to older adults with severe psychiatric disorders and includes a continuum of care, including treatment of dual diagnosis and homeless individuals. Under contract with the Department of Public Health, the programs provide clinic and field-based services to older adults needing specialty mental health services. The agency is committed to implementing Evidence-Based Practices and behavioral health research into its community-based mental health services.
Job Description
Under supervision of the Program Director, the clinical case manager participates in a multidisciplinary team, including master's level clinicians, peer specialists, interns, and psychiatric medication providers, serving as the lead point of responsibility for enacting client care. This position will be 50% in Intensive Case Management (ICM) and 50% in Geriatric Outpatient Services (GOS). The ICM caseload is capped at a lower level to allow for intensive services to stabilize individuals who are experiencing acute symptoms and related risk factors. The GOS serves clients who are stabilized and not experiencing a need for more ICM services but continue to need specialized outpatient services, including therapy. This position allows for smooth step downs for client care, as well as a mix of client caseload for the clinician. Both programs evaluate the needs of clients and develop comprehensive clinical assessments, treatment plans, and coordinate care among the team. Services may include individual and group therapy, crisis intervention, case management, client advocacy, and engagement strategies. Clinicians also maintain accurate detailed records of client contacts through Cal-AIM (Medi-Cal) and Medicare standards as well as county and state regulations. Additional duties as needed.
Specific Responsibilities
- Works closely with interdisciplinary team.
- Provides crisis intervention to prevent hospitalization, and if necessary, facilitates involuntary hospitalization.
- Performs intakes and assessments for older adults with psychiatric and dual diagnosis disabilities in a Felton clinic, community, and/or in their homes.
- Provides individual or group therapy to decrease mental health symptoms and help clients reach recovery goals.
- Provides case management services, including evaluations, service plan development, assisting clients in accessing housing, financial, medical and other needed services. Coordinates with other service providers.
- Maintains clinical records of each client contact or service, in accordance with mandated standards of Medi-Cal, Medi-Care, and San Francisco’s Community Behavioral Health Services.
- Participates in team meetings and case conferences through presentation of cases, discussion of clinical issues and exchange of information and ideas.
- Answers a 24/7 after-hours crisis phone for approximately one week a month to support clients in crisis after hours (and earn an additional stipend).
- Commits to ongoing training, including developing specializations in evidence-based treatments and practices and the geriatric specialization of mental health services, as provided by the Senior Division and Felton Institute.
- Other duties and responsibilities as assigned.
Minimum Requirements/Qualifications
- Master's degree or higher in Social Work or other Professional Counseling Programs, and registered with active license with CA BBS, including ASWs, AMFTs, and APCCs.
- A minimum of 2 years of case management and mental health treatment for at-risk older adults, including co-occurring disorders (master’s level internship experience considered).
- Moderate to comprehensive understanding of Specialty Mental Health Services under the Department of Health Care Services definitions in addition to skills in assessment, treatment planning, and mental health services pertaining to individuals living with serious mental illness.
- Strong recovery model approach to care and ability to form strong therapeutic relationships with clients from diverse backgrounds and who often are underserved by traditional care models.
- Ability to effectively diagnose and make clinical rationales for psychiatric care utilizing the DSM 5 and other measures including social determinants of health and functional impairment screenings where appropriate.
- Actively practices cultural humility and promotes a warm and welcoming environment for culturally and gender diverse individuals.
- Excellent communication skills both in written and verbal formats.
- Ability to travel within SF to meet clients in a variety of community settings including SROs, hospitals, shelters, and other settings required.
- Ability to be a team player with a welcoming attitude.

Salary Range: $93,000-$105,000 Annually
